- Background: Heart failure is a global health problem. It is associated with significant mortality and morbidity and carries a huge burden on the patients and the health system. Heart failure is the leading cause of hospitalisation in patients aged 65 or more and is one of the leading causes of cardiovascular deaths. To improve heart failure outcomes, we need to improve our knowledge about this syndrome in our area which is regional and rural NSW. Aims: This thesis aims to define the mortality and readmission rates following heart failure hospitalization in Australia, assess the trends in heart failure admissions over 10 years in the Hunter New England region, describe the mortality and readmission rates following hospitalisation in the same region and identify the main two types of heart failure, compare the outcomes and to the adherence to the guidelines for treatment. Methods: We performed a systematic review and meta-analysis of heart failure outcomes following heart failure hospitalisation in Australia. Then we used data from the Cardiac and Stroke Outcome Unit in Hunter New England over 10 years. Then we analysed the data to assess heart failure hospitalisation. In addition, we measured the outcomes following heart failure admission and the trends of the outcomes over the study period. In addition, we extracted data at a patient level to compare heart failure outcomes, demographics and management between the main two heart failure types. Results: The thesis contains four major interlinking studies. Study one “Mortality and Readmission Following Hospitalisation for Heart Failure in Australia: A Systematic Review and Meta-Analysis”. The first study was used as well to compare the outcomes of our regional area with the rest of Australia. Study two “ Heart Failure Hospitalisations in the Hunter New England Area Over 10 years. A Changing Trend”. Study three “Outcomes following heart failure hospitalization in a regional Australian setting between 2005 and 2014”. Study four ” Patient Characteristics and Long Term Outcomes of Incident Heart Failure Admissions in a Regional Australian Setting”. Conclusion: This thesis highlights the burden of heart failure in the Hunter New England area which is regional NSW. It also identified valuable information for the future direction and intervention to improve heart failure outcomes.
